Job Description:


Coordinates transition planning and services for each student in compliance with NYS SED regulations, Medicaid service coordination linkage, and vocational and life skills assessments. Responsible for providing services and resources for students, parents, and staff in the areas of transition planning. Responsible for the establishment and coordination of vocational and community based learning opportunities and student placement.


Reports to the School Principal


Transition Duties

  • In conjunction with the classroom teacher, coordinates transition planning for each student as they age out of program in accordance with NYSED regulations
  • Ensuring linkage to and maintains contact with Medicaid Service Coordinator
  • In conjunction with the classroom teacher and therapists and family, coordinates and conducts vocational and life skills assessments to determine interests, strengths and needs. Assessments will be used to: write the Student Exit Summary; assist in determining community experiences while in school; determine the level and type of service the student will need post-graduation.
  • Participate in the secondary students’ Individual Education Plan ('IEP') meetings.
  • Ensure all documentation is in order for application to OPWDD Front Door.
  • Provide parents with information concerning community resources available to help in the transition of their child.
  • Provide linkage to other outside agencies as need arises.
  • Attend WNY transition committee meetings for information on new regulations and programs.
  • Chairs the internal transition meeting with staff from the school and adult services
  • Keeps an updated list of transitioning students, the MSC and current levels of support for the committee’s reference.
  • Works cooperatively with Employment Services to determine if referral to ACCES-VR is appropriate and makes referral.

Development of Vocational Skills & Abilities

  • Analyzes student’s vocational/life assessments, interests and behaviors to assist in determining work/community readiness, abilities, and preferences.
  • Provides written results of vocational/life skills assessments with specific recommendations for transition planning and IEP goal development.
  • Assists in the development of vocational curriculum.
  • Works with the classroom teacher and therapists to develop IEP goals and curriculum related to vocational/life skills.
  • Works with Teacher Assistant/Transition to teach student’s vocational and life skills within the school and at community based placements.
  • Accompanies students into the community and provides training and supervision as needed.

Development of Community Partners

  • Based on student’s abilities and preferences, establishes criteria for work/community site selection.
  • Based on the requirements of the work/community setting, establishes criteria for student for each location.
  • Conducts interviews with the students for entry into the program.
  • Provides instruction on interview techniques.
  • Matches student’s interests and abilities with criteria of each placement site and works with the classroom teacher, driver and teacher assistant/transition to schedule learning opportunities.
  • Ensures all Department of Labor Requirements for Students with Disabilities (Section 64c08) are met prior to placing the student at work site.
  • Develops training plans for each student and obtains signed agreements by the student and parents to participate in the program.
  • Networks with local business and community leaders to gain knowledge of the opportunities for students in the community and help develop opportunities for community integration
  • Regularly schedules site visits to ensure the student is benefitting from the experience.
  • Maintains ongoing communication and documents ongoing contact with community partners.
  • Maintains and encourages ongoing communication between the teacher assistant/transition, classroom teacher, student and family regarding the student’s performance in the community.

Staffing and Personnel

  • Responsible for the supervision, assignment, training, evaluation, and discipline of the Teacher Assistant/Transition and Driver.
  • Participates in the selection process of new hires.
  • Disciplines, trains and assigns subordinates.
  • Establishes and appraises performance levels of subordinates.
  • Monitors compliance with on-site and off-site policies and procedures pertaining to, but not limited to: incident reporting, quality assurance regulations, fire safety and employee conduct.
  • Constantly maintains high professional standards at work with regard to personal behavior and interactions with other staff, individuals, and outside agencies.
  • Provides for staff development and training, including orientation, in-service training and use of community resources.
  • Participates in professional development and community and inter-agency committee to remain current on changing trends and program requirements.
  • Provides consultation, supervision, and instruction to staff through individual and group conferences in order to assist staff in development of vocational and transition skills.
  • Participates in Agency planning and program development.
  • Participates in the development of Agency policies and procedures.
  • Assist the Agency with marketing, public relations and general efforts to increase community awareness of our programs and services.
  • Consistently complies with all Agency policies and procedures.
  • Exercises good judgment, thoroughness and tact.
  • Performs other duties, as assigned.

Experience and Skills:


  • New York State 19-A Certification required upon hire
  • Current knowledge of labor market trends and needs.
  • Knowledge and understanding of current federal and state education and child labor laws.
  • Experience with vocational assessment
  • Experience with transition planning
  • Demonstrated experience in the community placement of students
  • Experience with vocational coordination
  • Posses good time management skills.
  • Ability and desire to work as part of an interdisciplinary team.
  • High level of written and oral communication skills.
  • Ability to be organized and to delegate tasks when appropriate.
  • Proficient computer skills including Excel and WORD.
  • Ability and desire to work as part of an interdisciplinary team.


New York State Special Education Teacher Certification and 5 years teachers experience OR Master’s degree in related field and 2 years experience with adult services OR BA and 5 years’ experience in adult services.

Valid NYS Driver’s License. 19a will be required.

Successful pass NYS DOT screening for 19a.
