Job Description:

SUMMARY OF DUTIES

The Job Coach provides quality services and supports provided both on the job and offsite which result in job performance success, building quality relationships at work, fitting into the work culture, accessing natural supports, and job retention. Implements all vocational goals. Responsible for daily and monthly documentation for Supported Employment, Pathways to Employment and ACCES-VR.

REPORTING RELATIONSHIP

Reports to the Coordinator of Supported Employment.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Assists individuals in learning job tasks and assimilating to the work environment
  • Promotes and facilitates worker independence, natural supports and job accommodations between the person and their supervisor/coworkers
  • Facilitates communication between individuals and the employer regarding expectations, rules of conduct and safety
  • Provides interpersonal skill building both on and off the job
  • Provides work related instruction, as needed.
  • Monitors quality and quantity of individual’s work to identify areas of need to obtain or maintain employment
  • Applies problem solving techniques to increase community independence and assist individual in obtaining or maintaining employment
  • Communicates and coordinates schedule with the Supported Employment Team to ensure coverage and safety of individuals we serve
  • Assesses individuals in community settings to determine the individual’s skills, abilities, unique talents, while supporting the individuals to make informed decisions
  • Communicates with Care Coordinator and the circle of support as needed
  • Participates in meetings in regards to the development of the Life Plan
  • Reviews and Implements the Supported Employment Habilitation Plan and Individual Plan of Protection to provide quality services and documentation
  • Fosters a positive and supportive work environment for the individuals receiving services
  • Maintains documentation in a timely manner including, but not limited to, case notes, supported employment daily documentation, incident reports and monthly documentation in accordance with regulatory compliance and Agency expectations
  • Transports individuals receiving services as well as provides mobility training
  • Constantly maintains high performance standards at work with regard to personal behavior and interactions with staff, individuals receiving services, employers and outside agencies
  • Familiarity with health and safety requirements and regulations for all individuals receiving services
  • Demonstrates the ability to work independently as well as in conjunction as with other staff members
  • Requires off-site supports and/or meetings with individuals on weeknights and weekends
  • Participates in staff meetings, group conferences and in-service training and community activities
  • Completes and maintains mileage logs in a timely manner and in accordance with policies and procedures
  • Participates in agency in-services
  • Consistently complies with all Agency policies and procedures
  • Performs other duties as assigned.

Experience and Skills:

SKILLS & ABILITIES

  • High level of written and oral communication skills
  • High level of interpersonal skills
  • Knowledge of computer and ability to utilize word processing programs, preferred
  • Possess good time management skills
  • Successful completion and certification of First Aid and CPR training, provided upon hire
  • Ability to work independently within the community

EDUCATION & TRAINING

Preferred: Associates Degree plus two years experience in production or manufacturing, and/or work with the developmentally disabled population.

Minimum: High School Diploma or Equivalency.

Valid New York State driver’s license in good standing, New York automobile insurance, and a reliable motor vehicle required.
